1. Each participant MUST complete an appropriate Medical and Liability Release Form.  For those participants under the age of 18, the student form MUST be completed by the parent or legal guardian who MUST complete the Parent/Legal Guardian portions of this form (Please note, for all under the age of 18, page two must also be signed and notarized by an official notary public, Canada – Commissioner of Oaths).
   
   
2. The participant completes the required Medical/Liability Release (child/youth must be notarized) form which may be filled-in or downloaded from the website.
   
   
3. Participant returns the completed, printed copy of Medical/Liability form to their Quiz Sponsor (this can be the regional or district director, local church quiz coach or other person chaperoning the participant to and from the event) or it may be emailed or faxed to the NYI office.  In any case, the Quiz Sponsor must keep a printed copy during the trip to and from the event.
   
   
4. The Quiz Sponsor or participant parent/guardian scans the Medical/Liability Release form and emails it to the NYI office at twagoner@nazarene.org.  The file must be labeled: District, Team Name - Last Name, First Initial (for example: ChicagoCentral_ONUA_HorningB.doc).  If a district or team has duplicate Last Names, First Initials, please use full first names to distinguish between the two.  The Quiz Sponsor retains the original form for traveling and onsite purposes.

·         All scanned or faxed copies of the Medical/Liability Release forms mus t be submitted to the NYI office before admission is granted to the event. 

·         Please note that online registration ends at Friday, May 28^th,(some late registrations may be accepted) 2010 and forms must be on file before the event (June 28, 2010) 

 This liability release is valid during Q2010, June 28-July 3, 2010
